I am not sure if I fully understand your question.
In addition are you talking about H.323 or SIP Video ?
A video enabled SIP call simply advertises additional SDP values in the SIP Invite. If the far end or the server understand and supports these the video call is established.
You can also simply call the IP address of another Video enabled endpoint.
