Is UC software 4.1 for IP phone 331 need license to activated, because when deploy with provisioning server, it request file (mac-address)-license.cfg.

welcome to the Polycom Community.
Our phones always attempt to load either an individual or a site license.
The UCS 4.1.x Software requires a License if used with LYNC. If you attempt to download this from the support page you have to acknowledge a disclaimer explaining this.
All other features requiring a license are described in the matching release notes.

it is used interoperability with lync 2010.
is the Non-VQMON license unable to activated uc software 4.1 for production suite site lisence?
what is different individual license and site license for lync 2010 interoperability with ip phone 331?

Hello ,
If used fore LYNC and when downloading the UCS 4.1.x Software you have to agree to the disclaimer explaining the need for a license.
Please be aware of the following when utilizing UCS 4.1.0 or later:
The use of Polycom UC Software versions 4.1.X (“Software”) requires the purchase of a separate Software license for every device that will use the Software in a Lync environment. You may not install, access, or use the Software in a Lync environment on more devices than are listed on your license until additional licenses have been purchased and authorized by Polycom. These licenses should be purchased from the same company from which you purchased the devices. You may operate devices against a Lync server for trial purposes for up to 30 days without purchasing a license. Use of the Software is subject to the terms and conditions of the End User License Agreement.
Polycom reserves the right to audit your deployment to verify that you have sufficient licenses to match the number of devices being used.
