I beleive the BootROM is 4.1. I used the split version since the BootROM was > 4.0 which is what I believe the Support Site instructed.

as mentioned below you may have configured multiple options via different access methods so my suggestion is to Format the File System and ensure the Phone is not loading any configuration files if you have setup your own Provisioning Server.
Any further support would need to be via your reseller or via Polycom Support (charges may occur) if the provided detailed steps in my previous post will not get you any further.

----------------The core issue was the the Session Manager was a part of the Avaya ME bundle and had set up many parameters automatically. I discovered that the automatic listening ports were TCP/5060 and TLS/5061. Once I added UDP/5060 all IP6000 sets came up without issue.
