i would like to setup my HDX 8000 with Lync however i keep getting errors. can you guys review the pictures and let me know what i am missing

You posted this in the wrong section and I will move this post shortly.
In addition it is always useful to include the currently used software version as issues experienced may already be addressed in a newer release.
This also allows yourself and others to check against current software release notes.
Is this Office 365 Multi-Tenant or dedicated ?
In addition Jeff's example uses the full account name including the domain where yours just uses the username.

i have tried both full account name and username
my Polycom software version is Release - 3.0.6-37004

to my knowledge you require HDX release 3.1.0 or later in order to be able to utilize Office 365 dedicated.
Please work with your Polycom reseller on the details of this.
