To manage a device using Poly Lens, does the device need to be in warranty, or have Poly+ associated with it? Can Poly Lens be used to manage devices without Poly+? i.e. are there any costs associated with Poly Lens or is a free service for Poly devices/customers?

Poly Lens is totally for free. You can mangage, configure and monitor your hardware with it without any cost. If your devices are under Poly+ support you have additional premium fuctions within Lens.
